Should be a standard 1/4″ , open frame mono jack.
Should be close enough:

Matt: The one I linked should work fine. From the photos, yours is a standard 1/4″ mono jack. I suspect that these were added as an afterthought, as I can’t find them in the IPB for any version of the L-5. If there is some info on them, it would have to be on the 32201 or 32231 drawings.
When I tore mine down, it had different jacks on each side, so I suspect that whatever was available at the time was what got used.

Just found out that AirCorps’ parts side of the house has the GJ-1 jacks available now.
